A ready-made digital survey form for running focus group discussions in cholera and acute watery diarrhoea programmes. It is an XLSForm, meaning the spreadsheet is uploaded to a mobile data collection platform such as Kobo Toolbox or Open Data Kit, and the questions are then answered on a phone or tablet.
It covers water sources, sanitation, hygiene, how people get and trust information, what people know about cholera, where they go for treatment, and the oral cholera vaccine, with optional sections on water committees, water vendors and food vendors. It also includes a section for recording who took part.
